All registrations are by appointment only.  However, we do understand that situations arise.  We ask that you please call ahead of time if you are going to be tardy or need to reschedule your appointment to a later date.  

Required Documents

BEFORE SCHEDULING YOUR REGISTRATION APPOINTMENT, YOU MUST HAVE ALL THE REQUIRED DOCUMENTS.  YOU CANNOT REGISTER A STUDENT WITHOUT THEM.

DOCUMENTS YOU WILL NEED IF:

OUT-OF-STATE TRANSFER OR FIRST TIME ENTRY

  • Student Entry Form
  • Emergency Information Form
  • Previous school records indicating courses and credits earned
  • Florida Physical                                  (Unable to Register Without – Per Florida State Law)
  • Immunization Records – Fla. Form 680
  • Birth Certificate
  • Social Security Card
  • Florida Driver’s License/Identification Card stating current address
  • 2 Forms of Residential Address (REQUIRED) Examples to follow:
    • Valid Lease Agreement
    • Deed To The Home 
    • Utility Bill (Electric, Water, Cable).
  • Court Order Custody Agreement (if applicable)

IN-STATE TRANSFER

  • Student Entry Form 
  • Emergency Information Form 
  • Previous school records indicating  courses, credits earned, and immunization records.
  • Birth Certificate
  • Social Security Card
  • Florida Driver’s License/Identification Card stating current address
  • 2 Proofs of Residential Address (REQUIRED)  Examples to follow:
    • Valid Lease Agreement
    • Deed To The Home
    • Utility Bill (Electric, Water, Cable).
  • Court Order Custody Agreement (if applicable)

IN-DISTRICT TRASNFER

  • In-District Transfer Form
  • Emergency Information Form 
  • Withdrawal Form documentation from previous school
  • Florida Driver’s License/Identification Card stating current address
  • 2 Proofs of Residential Address (REQUIRED)  Examples to follow:
    • Valid Lease Agreement
    • Deed To The Home
    • Utility Bill (Electric, Water, Cable).
  • Court Order Custody Agreement (if applicable) 
If you have all the documents except immunization records or transcripts, you can still register your child in the meantime.  These two documents are the ONLY exception.  ALL other documents are required for registration.
If you live with someone else and do not own or lease your residence, a NotarizedResidential Affidavit” form needs to be completed.  The homeowner’s Driver’s License AND electric/water bill MUST be attached.  You will have to provide ONE ADDITIONAL  proof of residency for yourself.
